Is this a unique Creamware cable or is there a readily avalable substitute?

no substitutes available, as the connector distance has to be as short as possible.
it's easy to diy if you have an electronics supply shop in the area (for the parts) and a wrench at home to press them together.
on the other hand it's not that simple (if it's your first custom made cable) that the price isn't justified
