Hey all, ends up it was due to the contacts.

Bought the caig deoxit & pro gold kit and cleaned all the contacts on all my cards - pci & stdm contacts and module pins. The cottons buds i used ended up being all black!! Dust or oxidization? Who knows, it's clean now, the dodgey ADAT and midi ports ...
